The Institute for Public Health (IPH) used the Dignity Health/Truven Health Community Need Index (CNI) to identify vulnerable communities within San Diego County. The CNI identifies the severity of health disparity for every zip code in the United States and demonstrates the link between community need, access to care, and preventable hospitalizations. CNI scores are assigned to each zip code based on the following five factors:
- Culture and language
- Educational levels
As part of the 2013 HASD&IC CHNA process, GIS maps were created based on the CNI data for each of the six regions. In addition, maps were created overlaying hospital discharge data for three conditions: Cardiovascular Disease, Type 2 Diabetes, and Behavioral Health. Please see below for the maps of each region.
