Exodus Recovery

The Exodus Recovery philosophy of treatment is grounded in a strong belief in the Whole Person Care model. Exodus promotes an innovative treatment model that integrates physical health, mental health and substance use disorder treatment in order to foster hope, empowerment, self-sufficiency, and recovery to vulnerable high utilizers of community services. Since 1989, Exodus Recovery has worked to design and develop models that have fundamentally changed how health care and social services are delivered, thus enriching and reinforcing the communities we serve. The Exodus model seeks to balance quality, cost efficiency, and positive client and community experience-all key components of a high performing health organization.

Over the course of thirty-one years, Exodus has developed a client-focused, recovery-based system of care that encompasses Psychiatric Health Facilities, Full-Service Partnerships, homeless outreach, and interim housing, mobile health, Wellness Centers, crisis transition and care coordination teams, forensic services, crisis stabilization/urgent care centers, telehealth services, substance use disorder programs, a sobering center, and programs for individuals with Intellectual/ Developmental Disorders. To date, Exodus Recovery has planned, implemented and operationalized programs within five California counties: Los Angeles, San Diego, Fresno, Napa and Orange Counties. Exodus currently holds contracts with Department(s) of Mental/Behavioral Health, Department(s) of Health Services/Health and Human Services Agency, Public Health, County Probation, The Regional Center, and the Center for START services. These efforts have created a continuum of care that includes approximately 1,000 employees and 43 programs and that has fulfilled the strategic objectives of reducing utilization of health systems, medical emergency rooms, law enforcement interventions, limiting hospitalizations, reducing homelessness, and creating safe and secure environments where those with complex conditions can receive compassionate and caring specialized services.

Grossmont Healthcare District

Helping our community address its unmet healthcare needs.

In 1952, the Grossmont Healthcare District – known at that time as Grossmont Hospital District – was formed to build and operate Grossmont Hospital. In 1991, the District Board decided to lease the hospital operations to Sharp HealthCare. The affiliation agreement included the establishment of a non-profit entity, Grossmont Hospital Corporation.

Today, Grossmont Healthcare District continues to serve as landlord of the hospital on behalf of local taxpayers and is a public agency that supports many non-profit, health-related community programs and services in San Diego’s East County region. In addition to assisting these non-profit organizations, the District also is an active supporter of healthcare education and training for the benefit of future generations. The District operates the Dr. William C. Herrick Community Health Care Library at Briercrest Park, a comprehensive consumer health library specializing in providing reliable health information, accessible both on-site and online. The District also operates the James G. Stieringer Conference Center and smaller meeting rooms that are available to community-based, non-profit, health-related organizations for lectures, seminars, workshops, and classes.

Since its inception in 1987, Hooper, Lundy & Bookman’s primary focus has been to guide and support health care providers and suppliers in our shared mission to create and maintain a viable and effective health care system. Without that, people, especially the most vulnerable among us, face significant challenges in accessing the quality health care necessary to keep our communities healthy, safe and thriving. We are passionate about helping our clients to fulfill this mutual goal of serving all populations.

We believe that the complexity of health care law requires a depth of understanding that goes beyond that commonly found in general practice firms with health care law departments. Such depth can only be achieved through industry specialization and experience. That is why we have dedicated our practice exclusively to understanding the intricacies of the health care industry. Our attorneys and government relations specialists represent providers and suppliers in all aspects of their operations, including business and regulatory transactions, litigation, arbitration and dispute resolution needs, as well as advocacy and public policy. The firm has represented clients in many of the landmark appellate cases that have shaped health care law over the past two decades.

Hooper, Lundy & Bookman has developed a COVID-19 Resource Center, which includes links to federal and select state resources on COVID-19, guidance from select national trade associations, and alerts and webinars on select topics of interest for the health care provider community. vitas-healthcare-png

VITAS Healthcare has proudly served the San Diego community since 1978, by providing care to those with advanced illnesses and their families. We believe delivering great hospice care starts with providing access to more services designed to meet the needs of our patients, families and referral partners.

In 2018, our home medical equipment team provided 1625 stat equipment deliveries for faster acute setting discharges. Our staff had the privilege of providing over 104,000 days of care for our patients in San Diego. Our Vitas physicians continue to make an average of 100+ face to face visits to our patients each month.

VITAS Healthcare transitions patients efficiently and seamlessly from hospital to home, from curative to palliative, from chronic to end-stage. As a post-acute care provider, the VITAS program in San Diego provides advanced levels of care, including, when medically necessary, shifts of Intensive Comfort Care (continuous acute symptom management) in the home and, when symptoms cannot be controlled at home, round-the-clock inpatient care through our partnerships with 17 local hospitals and more than 80 skilled nursing facilities. Our hospice services reduce the likelihood of futile visits to the ED and unnecessary hospital admissions.

The VITAS team in San Diego is one of 44 offices nationwide serving patients in 15 states and the District of Columbia. At VITAS, we make sure that patients and their families come first!

Hospice Care During the COVID-19 Pandemic
VITAS Healthcare remains a committed provider and partner during the COVID-19 pandemic.
