Hospitals and health care systems in San Diego County have a long history of responding to health needs in the communities they serve. This commitment extends beyond traditional hospital care to community health programs that provide services to the region’s most vulnerable populations. Community health programs are created based on an assessment of needs identified through hospital data, community input, and major trends (national, state and local statistics). Previous collaborations among not-for-profit hospitals and other community partners have resulted in numerous well-regarded Community Health Needs Assessment (CHNA) reports.
The objective of the Hospital Association of San Diego & Imperial Counties CHNA is to identify and prioritize health issues and health needs in San Diego County.
The CHNA Committee is now planning for the 2022 collaborative CHNA. If you are interested in providing feedback, participating in the process, and/or sharing data from your organization, please provide feedback through this online survey form below or contact us. We appreciate your time in completing this survey and contributing your input into our CHNA process.
