Associate Member Spotlight: 211 San Diego/CIE/San Diego Health Connect

211 San Diego, Community Information Exchange©, and San Diego Health Connect embarked on a groundbreaking cross-sector journey in early 2024, embracing a unique partnership that gives 211 San Diego executive oversight of the region’s health information exchange.

As the nation’s only 2-1-1 system that combines community and health information exchanges, this model unites essential pillars of the San Diego region, transforming the landscape of person-centered and holistic care. Motivated by a desire to address the social determinants of health, as well as to create a more comprehensive and integrated health and social services network in the San Diego region, the combined agencies leverage their strengths to provide a wider range of services to a larger number of people and improve the community’s overall health and well-being.

211 San Diego is a local nonprofit organization whose mission is to seamlessly connect people with the community resources and health services they need, and to partner with the community to transform how people access help.

The Community Information Exchange is a community-governed infrastructure that enables effective and responsible information sharing among many organizations to improve the health and well-being of people across San Diego County.

San Diego Health Connect is the health information organization that unifies the San Diego health care ecosystem and securely connects providers, patients, private health information exchanges, and others to improve the quality and cost of care in the community.
