Community Health Group, founded in 1982 by a Federal Qualified Health Center, is the only San Diego-based, not-for-profit managed care plan (MCP) serving California Medi-Cal beneficiaries. Today, we serve nearly 360,000 members throughout San Diego County. As a local plan, we live in the same communities as our members, and we understand the unique needs of those communities. We are dedicated to serving diverse populations; our workforce is a reflection of the diversity of our members. We have also built close relationships with our partners, including our providers, hospitals, and key stakeholders. These partnerships have shown us both the existing community assets and the challenges for change.
