5 Things To Know: Save the Date for Rural-Focused PATH Cited Round 4 Grant Applications

The Providing Access and Transforming Health (PATH) enables provider participation in the Medi-Cal delivery system — and its Capacity and Infrastructure Transition, Expansion and Development (CITED) Initiative provides funding to enable the transition, expansion, and development of enhanced care management (ECM) and community supports (CS) capacity and infrastructure. In limited circumstances, organizations that do not directly provide ECM and/or CS but support the delivery or administration of these services may also apply for CITED funding starting Jan. 6. The final PATH CITED Round 4 priorities include ECM/CS in rural counties and county-specific gaps in ECM by populations of focus. The average award in the first three rounds was $1 million.
